๐ Plena Health: The Back Office That Runs Itself
A YC-backed AI "operating system" that runs the back office for specialty clinics โ end to end.
The first clinic to run Plena belonged to the founder's mom. After three years embedded in specialty practices, Eyad Abdalla kept hearing the same plea from the front desk โ not "give us better software," but "please, just clear the fax queue" โ so he built the AI that does. Let's check them out!
โพ๏ธ The Elevator Pitch
Run a specialty clinic and the real grind isn't the medicine โฆ it's the back office. Referrals still arrive as faxes, prior authorizations die in payer phone trees, schedulers chase no-shows by hand, and billers fight down-coded claims across a dozen systems that don't talk to each other. U.S. healthcare burns roughly $950B a year on administration, and McKinsey pegs about $265B of it as pure waste. Meanwhile the squeeze is getting worse: practice operating expenses jumped 11.1% last year, and 41% of providers now see more than one in ten claims denied.
Enter Plena Health, a Y Combinator (Spring 2026) startup building what it calls the "AI operating system for specialty medical practices." The pitch is right there in the tagline: while you see patients, we handle everything else. Plena's AI agents sit on top of a clinic's existing tools and quietly do the rote work end to end โ reading every inbound fax and matching it to the right chart, answering booking calls and backfilling cancellations off a waitlist, chasing prior auths through portals and IVR phone trees, and scrubbing claims before they ever go out. Founded in 2025 by Eyad Abdalla (ex-Datadog and Shopify) and Ahmed Al Mudarris (who built tier-zero infrastructure at AWS) โ both sons of physicians โ Plena has grown 17ร in roughly nine months to seven-figure committed ARR, with about 80% of it coming through word of mouth.

๐ Founded: 2025
๐ฅ Founders: Eyad Abdalla (CEO), Ahmed Al Mudarris (CTO)
๐ฐ Stage: Backed by Y Combinator (Spring 2026) โ no outside round publicly disclosed
๐ Traction: 17ร growth in ~9 months ยท seven-figure committed ARR ยท 6 specialties across 8 states ยท 40+ EMR integrations ยท ~80% word-of-mouth
๐ฎ Tech Trend: Agentic AI for the healthcare back office
๐ฏ Target Market: Independent specialty practices โ derm, GI, ortho, ENT, ophthalmology, urology
๐ข Location: San Francisco, CA
๐ Why We Like It
๐ The math finally works: The back office Plena automates is the global revenue cycle management market โ about $343.8B in 2024, on track for ~$894B by 2033. What changed is cost: as reasoning-model prices collapsed (o3 fell ~80% in two months), pointing agents at fax queues and payer portals went from absurd to economical. With admin costs at record highs and reimbursements shrinking, clinics finally have both the pain and the tooling to automate.
๐ It moves in โ it doesn't rip things out: Most health-tech asks a clinic to migrate to a new system and retrain its staff. Plena instead runs on top of the 40+ EMRs practices already use โ Epic, athenahealth, Oracle Cerner โ as an "operating layer," not another tab to babysit. One practice's engineer "learned our workflow in two days," and a dermatology group used Plena to clear a 4,800-referral, four-month backlog without adding headcount. That low-friction wedge is why ~80% of growth is word of mouth.
๐ซ Operators who lived the problem: Abdalla spent three years inside specialty clinics before concluding they "don't need more software โ they need someone to do the work," then paired with Al Mudarris, who kept the systems that "can't go down" running at AWS. Both grew up as doctors' kids, and Eyad's mother's clinic was Plena's very first user โ a rare mix of front-desk empathy and infrastructure-grade engineering.
The challenge will be the crowd. Plena is charging into a well-capitalized field: Tennr raised a $101M Series C at a ~$605M valuation for referral automation alone, while Commure, Notable, and Adonis are all chasing the same back office with far bigger war chests. The deeper risk is the exception rate โ every fax or claim the agents can't resolve cleanly just shifts labor rather than deleting it, and 40+ integrations are a maintenance load the EMR vendors have little reason to keep stable. Staying the most accurate, and the easiest to live with, is the game.
